Home And Community Care Services (HACC)

Home and Community Care Services are government funded services that support frail older people, or people with disabilities, so that they can live in their own homes and communities rather having to move into a nursing home or hostel.

HACC Services also assist those who care for frail older people, or people with disabilities, at home.

  •  Help - with shopping, housework, personal care
  • Food - Home delivered meals
  • Transport - transport to go shopping, to medical appointments or on social outings
  • Respite Care - giving carers a rest from their caring role. Support is provided at home or in a social setting.
  • Home Nursing - visits by a trained nurse
  • Community options - helps people with multiple needs
  • Dementia support - help for people with Dementia or memory loss, or those that care for them
  • Allied Health Services - services such as physiotherapy, and podiatry(foot care)
  • Home maintenance and modification-help with installing grab rails, and essential home repairs
