Community Options Program provides brokerage and case management services to prevent inappropriate admission to long term residential facilities. The service is provided to the frail aged and people with a functional disability with complex needs, and their carers. The program operates on a non- medical model, using multi-disciplinary staff to provide case management and brokerage services to support the individual person and their carer within their own community. Support is provided in a flexible manner using a person centred approach.
