Community Aged Care Packages (CACP)

A CACP is an individually tailored package of care services which helps older people, with lots of different care needs, to stay living at home by co-ordinating the different services they need.

You have to have an assessment first by an Aged Care Assessment Team. Then if a CACP is appropriate for your needs, you will be referred to a service coordinator.

In most cases, you will be charged a fee for your Community Aged Care Package. If you are on the basic pension, you cannot be asked to pay more than 17.5% of that pension.
