Richmond Valley Aged Care Assesment Team
The primary purpose of the Aged Care Assessment Team (ACAT) is to undertake multi-disciplinary, comprehensive assessments of frail older people with a view to assisting them to remain in their own homes as long as possible, whilst promoting their health, wellbeing, dignity and independence.
To achieve this goal ACAT aims to ensure the frail older person is able to gain access to available care services appropriate to their needs, and improve coordination of aged care services with other health and community support services.
Members of ACAT are Commonwealth Delegates for the authorisation of admission to Residential Aged Care Facilities and for the receipt of Community Aged Care Packages, Extended Care at Home Packages and Transition Care.
