External Entities Agreement
An External Entitiy Agreement is required to be completed when a Human Research Ethics application does not involve NCAHS patients, staff or facilities. It requires completion and sign-off by both the external entity and the NCAHS delegate.
It is requested that the External Entity contact the Research Ethics Officer on 02 6588 2941 or EthhicsNCAHS@ncahs.health.nsw.gov.au prior to submitting an ethics application. An individual External Entity Agreement will be drawn up for each individual applicant.
The External Entity should complete the blue section on page 1 and on the signature page, sign and have witnessed accordingly. (Typying should be in black font. ) The document should then be sent to:

The Agreement will be signed by the NCAHS delegate and a copy returned to the External Entity.
