Claim/Payment Forms:
(For details as to when these forms should be used refer to "Reimbursement of Expenses" in the Guidelines)
Claim Form 1 for community representative attending approved meeting and claiming set fee
Claim Form 2 for miscellaneous expenses (eg telephone calls) incurred as a community representative
Vehicle Insurance details confirmation form for community representatives using personal vehicle to attend meetings and claiming per kilometre reimbursement
Electronic Funds Transfer Form for community representative to receive payments direct to bank account


Any queries relating to the engagement of community representatives or the process for payment/reimbursment of community representatives should be directed to the NCAHS Community Participation Unit.
