Referral To the Multidisciplinary Pain Management Clinic
All Patients must be referred to the Multidisciplinary Pain Management Clinic by their GP or medical specialist
A copy of the referral form can be downloaded from here
Referrals to the Multidisciplinary Pain Management Clinic should be accompanied by:
- Copies of relevant specialist reports
- Copies of investigations i.e. radiology reports (x-rays, CT scans, MRIs etc)
- Current care plans from the treating doctor
- A copy of the current medical certificate (in compensable cases)
Once this has been received a booklet will be sent to the referred patient to collect further relevent information. upon receipt of the completed booklet, patients will be placed on to the waiting list for a multidisciplinary assessment.
It is important to ensure prompt return of this information.
