When It's Time To Go Home
Discharge.
Before you leave the hospital, make sure that you, your spouse or partner, relatives and friends know how to care for you at home.
Please make sure that your doctor has explained any new medications to you, including anything of which you should be aware such as medication administration instructions, diet, possible complications etc.
Check with your nurse about any necessary follow up appointments.
Please remember to collect any X-Rays and medications brought in with you on admission.
If you believe that you will require any sort of community care or assistance when you are discharged, then please discuss this with the nursing staff. They will refer you to the relevant community services if necessary.
In most instances patients will be discharged in the morning. Please make arrangements with relatives or friends to collect you. Should there be any problems with your discharge time or transport home then please discuss this with the nursing staff.
