Complaints

Lismore Base Hospital welcomes your feedback about the hospital.

If you wish to enquire about any aspect of your treatment at Lismore Base Hospital or give feedback or make a complaint, you can

· speak with a Nurse Manager at the hospital or

· contact the Quality Coordinator on 6620 2209 or

· write to Lismore Base Hospital, PO Box 419 Lismore 2480

After your complaint has been received by the hospital, you will receive a letter of acknowledgement from the Executive Officer to inform you that your complaint is being investigated.

If you have made a complaint on behalf of another adult, that person will need to sign a form Lismore Base Hospital sends to you to give permission for Lismore Base Hospital to investigate the complaint and inform you of the results of that investigation.

After the investigation has been completed, you will receive a letter from the Executive Officer informing you of the results of the investigation.

If you wish to discuss your concerns with a Patient Support Officer in the Health Care Complaints Commission, you can phone the Lismore office on 02 6620 7663 or toll free 1800 043 159. The Patient Support Officer is based in Lismore and available to talk with you about your concerns about your health care services.
