Position Description

Purpose of Role:
Provision of pharmacy services at Lismore Base Hospital.

Role Responsibilites:

  • Provision of clinical pharmacy services including: a. Medication reconciliation b. Monitoring drug therapies c. Counselling and educating patients d. Documenting interventions in medical records e. Preparing medication profiles on discharge.
  • Dispensary activities including supervising and checking pharmacy assistants and technicians, provision of methadone services, S4D and S8 dispensing and stock accounting.
  • Provide drug information and education to a range of stakeholders.
  • Supervision of pre-registration graduate pharmacists and undergraduates undertaking clinical placements.
  • Participate in the Pharmacy Department continuing education program.
  • Support other staff, including preceptors/supervisors of pre-registration graduates, in developing and delivering pharmacy education to the relevant staff.
  • Participate in Pharmacy Department quality program, including the evaluation of educative processes and outcomes.
  • Assist and support other staff with the development of procedures/guidelines in relation to clinical pharmacy.
  • Other duties consistent with the Award or Terms of Employment as may be directed by Director of Pharmacy, or implied from time to time.

Essential Criteria

  • Registration as a pharmacist in NSW or ability to obtain registration by the time of commencement with NCAHS.
  • Previous experience in hospital pharmacy.
  • Demonstrated knowledge of contemporary clinical pharmacy practice.
  • Demonstrated ability to communicate effectively with all stakeholder and to function as an effective member of a workplace team.
  • Ability to use judgement, initiative and independence in problem solving in clinical pharmacy.
  • Ability to participate in teaching programs with other Pharmacists and/or university students, community health, nursing staff or other health care providers.
  • Demonstrated commitment to Continuing Professional Education.
  • Ability to participate in an on-call and weekend roster.
