Position Description
Purpose of Role:
The primary objective of Aged Care Assessment Team is to assess and where appropriate reassess the physical, medical, psychiatric, psychological and social needs of consumers, to focus on potential restoration of functional ability, identify their care needs and in consultation with consumers and significant others help them to choose the most appropriate available services to meet those needs.
Role Responsibilites:
- Adopt a holistic approach to the assessment of elderly clients in relation to their physical, intellectual, emotional and social needs.
- Arrange with the service users agreement or that of an advocate, the prompt provision of available, appropriate services to meet the assessed needs.
- Educate clients and carers in the maintenance of function and encourage a satisfying level of independence within the client’s capabilities.
- Attend assessments of aged persons in the community, Residential Aged Care Facilities, and public and private hospitals upon referral.
- Undertake follow up of clients where necessary in order to review needs. Organise any necessary alterations to ensure continuity of preferred lifestyle where possible.
- Consult within and across disciplines in order to achieve a consistent, coordinated approach to care.
- Participate in planning in the area of aged care.
- Maintain appropriate client records and statistical data as required by the Health Service
- Perform other duties not inconsistent with the Award, Agreement or Terms of Employment as may be directed or implied from time to time
- Assist carers to overcome anxieties in relation to the caring role. Arrange available appropriate respite on request.
- Utilise an advanced level of skills, knowledge and proficiency in the delivery of care to elderly clients.
- Act as a client advocate by ensuring the rights, safety, privacy and confidentiality of clients is maintained.
- Liaise with medical, paramedical, hospital staff, community health staff, community service providers, residential aged care providers and act as alink between acute and community care.
- Attend hospital and community meetings as appropriate.
- Participate in orientation and other staff development activities.
- Demonstrate relevant proficiency in the use of Information Technology.
- Practice in accordance with professional code of conduct, ethics, and legal parameters.
- Monitor the work environment to identify workplace health & safety hazards to promote staff well being, effectively managing risk.
- Assist with strategies to match cost centre expenditure to budget allocation.
- Maintain caring and effective interpersonal relationships with clients, carers, and staff to provide optimal quality care within a co-operative environment.
- Work within the Aged Care Assessment Guidelines.
Essential Criteria
- Registration with the Nurses and Midwives Board (NSW) or ability to acquire same under the Mutual Recognition Act.
- Extensive experience in aged care nursing.
- Demonstrated experience in the assessment of an aged person's physical, medical, psychological and social condition, function and needs, including an understanding of the common diseases of old age.
- Ability to provide education and self management information to client's relatives and carers and ability to provide support and direction to staff working with aged persons.
- Demonstrated effective interpersonal and communication skills.
- Demonstrated understanding of the principles of case management/care coordination.
- Demonstrated commitment, and ability, to work within a multi-disciplinary team and with a range of personnel, including general medical practitioners, and other agencies.
- Current NSW Drivers licence.
