Position Description
Purpose of Role:
The focus of Social Work input is to address the psycho-social aspects relating to patient's health care.
Role Responsibilites:
- It is the responsibility of each staff member to be aware of the contents of the Policy and Procedure manual/s for their Department and work within the principles contained therein.
- To provide a Social Work service to inpatients and occasionally outpatients of the Coffs Harbour Health Campus.
- To complete psycho-social assessments, plan and implement intervention, relating to the health presentation of the patient.
- To perform complex duties of case management with patients, including family work as appropriate, relating to the health issue, within relatively short time frames.
- To participate in discharge planning for patients, alongside and in consultation with other disciplines.
- To provide short term counselling to patients and their families to assist them in coping with illness, hospitalisation and / or social issues associated with their current situation.
- To provide advocacy and liaison with other seervice providers.
- To develop, facilitate and evaluate group sessions as appropriate.
- To actively engage in the review of the Social Work service. To initiate, identify, implement and evaluate areas of quality improvement, from principles of evidence based practice and professional development, that will enhance patient outcomes.
- To actively identify and engage in actvities that seek feedback on SW service from patients.
- To attend relevant hospital / Social Work meetings to ensure shared information and positive communication.
- To maintain records and statistics in accordance with North Coast Area Health Service and Health Department procedures.
- To actively participate in regular line management sessions with a Senior Social Worker, participate in supervision and engage in professional development activities.
- To be aware of clinical pathways and guidelines for working with particular client groups eg stroke patients.
- Perform other duties not inconsistent with the Award or terms of Employment as may be directed or implied from time to time.
- To rotate across service areas that are within the jurisdiction of the Social Work Department, as required by the Head of the Social Work Department.
Essential Criteria
- Appropriate tertiary qualification in Social Work providing eligibility for membership of the Australian Association of Social Workers.
- Demonstrated knowledge of Social Work role in the context of inpatient service in a hospital.
- Demonstrated ability to organise and facilitate family meeting and co-ordinate a case management plan.
- Demonstrated ability to provide a range of Social Work interventions, including:-group work, short term counselling, advocacy, liaison, networking, referral, conflict resolution, case co-ordination and discharge planning.
- Demonstrated commitment and ability to work within a team approach, alongside other disciplines.
- Demonstrated ability to complete psycho-social assessments, address a range of issues relating to health presentation and plan intervention as required.
- Identify, develop and evaluate quality improvement activities for Social Work, based on research and professional development principles.
