Position Description

Purpose of Role:
The MO will be the Director and Lead clinician in the provision of the Community Acute / Post Acute Care Service (CAPACS), Streamed Fast Track model of care within the Emergency Departments under a private Billing arrangement(The Express Community Care Clinic) that would include a limited procedural day clinic service. The Career Medical Officer (CMO) works to the direction of the Director of the Emergency Department or present FACEM to perform 'fast track'. In relation to the CAPACS matters the CMO works in liaison with Physicians, Surgeons and General Practitioners

Role Responsibilites:

  • The efficient and effective delivery of patient services in accordance with North Coast Area Health Service and Port Macquarie Base Hospital Policies and Procedures, emergency and community needs and within budget allocation.
  • Full assessment and care of patients presenting to the ECCC and the Emergency Department. Effective discharge planning of all patients receiving Fast-Track and CAPAC services.
  • Effective working with all service partners including primary care, to ensure that integrated services are delivered that facilitate patient care and collaborative partnerships.
  • The provision of safe, appropriate, patient focused, outcome directed, coordinated quality care that meet professional standards and is based on best practice standards and appropriate referral and continuing care for all patients.
  • To provide the ongoing medical governance for all patients admitted to the CAPAC service in consultation with Physicians, Surgeons and General Practitioners.
  • To perform patient assessment and clinical examination of patients, both within the Emergency Department, inpatient wards and Ambulatory Care.
  • To be responsible to, and carry out the instructions of the Hastings Macleay Network Director of the Emergency Department when performing Fast-Track.
  • To oversee, receive and provide, detailed verbal hand-over between Medical Practitioners .
  • To work within the multidisciplinary team, including liaison with referring Medical Practitioners, hospital based community services, community Allied Health Services,and Non-Government Organisations.
  • To maintain close communication with the relevant inpatient Medical teams.
  • To participate in the multidisciplinary teams and to provide leadership in Policy and procedure development towards the delivery of patient care.
  • To comply with all legislative requirements under Occupational health & Safety, Privacy Acts, and EEO.
  • To conduct all activity in a professional & responsible manner
  • Perform other duties not inconsistent with the Award, Agreement or Terms of Employment as may be directed or implied from time to time.
  • SPECIFIC DUTIES COMMUNITY ACUTE / POST ACUTE CARE SERVICES (CAPACS)
  • Assess patients in the Emergency Department and inpatient Units to decide if care can be maanged at home.
  • Review of referrals from external agencies e.g. Aged Care Facilities, GP's on suitability on CAPACS.
  • Oversee and conduct in-centre CAPACS clinics for patients returning to in-centre for review.
  • To support acute or post-acute care in the community, maintaining appropriate liaison with the clients, GPs, and hospital VMOs.
  • Appropriate referral and planning for continuing care, including referral to Allied Health and support services.
  • Arrange re-admission to the Emergency Department or to hospital if required.
  • Provide medical support to other CAPAC Team members.
  • Ensure continuing patient care with completion of discharge summaries, referral letters, assessment and follow up care.
  • Work within the NCAHS framework of quality improvement and good clinical governance.
  • Compliance with NCAHS policies in all areas of clinical responsibility and local CAPAC procedures.
  • Involvement in the evaluation of the program.
  • EMERGENCY AND FAST TRACK
  • Direct care of patients identified through the triage process to be suitable for 'fast track' care - these patients will be typically be stable, low risk, or requiring procedures.
  • Contribute to maintaining patient flow through the emergency department.
  • Data entry into the Emergency Department Infermation System (EDIS).
  • Referral of patients to appropriate internal and external services (including MAU and CAPACS).
  • Work in collaboration with the Clinical initiative Nurse (CIN) and ASET and to provide oversight in conjunction with appropriate Nursing Authorities to Nurse Practitioners and other advanced Nursing roles.
  • ADMINISTRATIVE The MO will be expected to participate in administrative oversight functions in the ECCC.
  • Ensure that policies, procedures and practices relating to the care of patients requiring services are consistent with contempary best practice, Professional Colleges, NSW and National Standards.
  • Establish, develop and improve patient care guidelines for common diagnostic groups.
  • Monitor, report and assist in the investigation of critical incidents / adverse events.
  • Work within the constraints of the Emergency/ECCC/CAPAC budget.
  • To maintain appropriate medical records, including electronic documentation (patient care, referral and medication systems), to ensure effective communication.
  • Attend relevant meetings related to the Emergency and CAPAC services.
  • To facilitate service planning.
  • To meet all legal requirements.
  • Active participation in continuing medical education programs organised by Port Macquarie Base Hospital Emergency Department and Hospital wide educational programs.
  • Contribution to the education of other health care professionals at individual and group levels.
  • Supervision of nursing and junior medical staff and review of their patients as required.
  • Ensure participation in clinical quality activities as outlined in the clinicians toolkit i.e. Peer review and Quality Improvement processes.
  • Participation in research opportunities.

Essential Criteria

  • Medical Degree with current unconditional registration with the NSW Medical Board.
  • Specialist qualifications in General Medicine or Emergency Medicine or equivalent Experience.
  • Demonstrated Leadership skills in managing a multidisciplinary team.
  • Demonstrated competency in clinical Emergency Medicine and General Medicine.
  • Demonstrated ability to provide safe effective patient care, working independently and without direct supervision.
  • Demonstrated written and verbal communication skills.
  • Demonstrated ability to work in a multi disciplinary team.
  • Drivers Licence.
