The Aged Care & Rehabilitation team in the Central Network covers the Local Government Areas of Hastings Shire Council and Kempsey Shire Council. This area runs from as far north as Stewarts Point and as far south as Johns River. Our region east to west runs from the coastline out to Comboyne and Bellbrook.

Hastings/Macleay Aged Care Assessment Team
The primary purpose of ACAT is to undertake multi-disciplinary, comprehensive assessments of frail older people with a view to assisting them to remain in their own homes as long as possible, while promoting their health, wellbeing, dignity and independence. The program aims to ensure the frail older person is able to gain access to services appropriate to their needs and improve conditions of aged care services with other health and community support services. Members of ACAT are Commonwealth Delegates for the authorisation of admission to Residential Aged Care Facilities and for the receipt of Community Care Packages.

Hastings Valley Community Options Project
The Community Options Program provides brokerage and case management services to prevent inappropriate admission to long term residential facilities. The service is provided to the frail aged and people with a functional disability with complex needs, and their carers. The program operates on a non- medical model, using multi-disciplinary staff to provide case management and brokerage services to support the individual person and their carer within their own community. Support is provided in a flexible manner using a person centred approach.

Community Care Packages/ Housing Linked
The purpose of the program is to provide planned and coordinated packages of care services to help older people in the community with complex needs remain living in their own home. The packages are designed for each individual and are based on their particular needs. The packages are targeted at persons with financial hardship living in designated rental housing developments.

Aged Care Transitional Intervention Program
The Aged Care Transitional Program uses a case management model of service planning and delivery which provides timely, efficient and individualised care. The program provides services following an acute illness or period of hospitalisation, for those who would benefit from a short period of extra care in their home. Services provided include assessment, referral, nursing care and home based physiotherapy and occupational therapy.

Mobile Respite Service
The Mobile respite Service, for carers of people with dementia and challenging behaviours in rural and remote areas, is a 4 year pilot program funded by the Commonwealth Department of health & Aged Care. The purpose of the service is to provide flexible, cost effective; respite, education and support for carers through home visits for assessment, referral, education and provision of respite either directly by the team, or via brokerage funds.

Hastings/ Macleay Community Health Podiatry Service
The Podiatry Service is funded through the Home and Community Care program for frail aged and younger disabled people who meet the criteria. The aim of the service is to reduce the incidence of foot pathology amongst the target groups serviced by the clinics.
