Richmond Community Options
This service is specifically for people with disability, dementia or the frail aged. It operates within the Ballina, Byron, Lismore, Richmond and Kyogle Local Government Areas. It provides case management to people who are eligible for HACC funded services but with complex and multiple care and support needs.
A case manager is allocated who will perform a comprehensive assessment and assist the client to access all available services and resources. The case manager will maintain contact to monitor the care situation.
Clients are provided with case management for an agreed period of time. Clients and carers are included in all planning, including discharge planning.

Hastings Valley Community Options Project
The Community Options Program provides brokerage and case management services to prevent inappropriate admission to long term residential facilities. The service is provided to the frail aged and people with a functional disability with complex needs, and their carers. The program operates on a non- medical model, using multi-disciplinary staff to provide case management and brokerage services to support the individual person and their carer within their own community. Support is provided in a flexible manner using a person centred approach.
