Hastings Valley Community Options Project
The Community Options Program provides brokerage and case management services to prevent inappropriate admission to long term residential facilities. The service is provided to the frail aged and people with a functional disability with complex needs, and their carers. The program operates on a non-medical model, using multi-disciplinary staff to provide case management and brokerage services to support the individual person and their carer within their own community.
